GetWellNetwork provides the first digital platform to engage patients throughout the patient journey. The patient’s journey begins before entering the hospital and extends beyond discharge. Using the web, mobile phone and cable TV, GetWell@Home enables hospitals to engage patients throughout their journey. This patient-centered approach to care leads to improved patient satisfaction, lower cost per case and better compliance with Meaningful Use standards.

Improve Patient Satisfaction

GetWell@Home plays a key role in helping patients remain actively involved in their care. Activated patients have a better experience, better outcomes and therefore, better overall satisfaction. One of the biggest challenges hospitals face, particularly with the development of Accountable Care Organizations, is effective care transitions and post-discharge care. Patient Satisfaction is often negatively impacted when patients feel there has been incomplete or ineffective communication during and after their hospital stay. Patients can feel lost or abandoned. GetWell@Home provides patients the ability to stay connected to their care team while taking part in preventative care or managing their chronic condition. The result is a healthier, more satisfied, loyal patient.

Decrease the Cost per Case

GetWell@Home improves transitions into and out of the hospital. In preperation for admission or after discharge, patients access their care plan, review education, learn about their medications, schedule appointments, view lab results and follow-through on other self-monitoring responsibilities as prescribed by their physician.

As CMS outlines new pay-for-performance programs and in particular, the Medicare Bundled Payment Plan, hospitals will increasingly be responsible for the full cost of readmissions during the 30 days post-discharge. Patients that have better post-discharge care are likely to have fewer readmissions. The patient’s use of GetWell@Home can help prevent readmissions and as a result, reduce the hospital’s overall cost per case.

Satisfy Meaningful Use Standards

Compliance with Meaningful Use standards presents many new challenges for hospitals. Standards for the effective use of electronic health records are being introduced over three stages from now until 2015. Hospitals will be required to document and demonstrate consistent compliance with these standards in both the inpatient and outpatient settings. Hospitals that are not in compliance face potentially significant reductions in Medicare reimbursement each year.

GetWell@Home can help hospitals satisfy the current Stage 1 Standards. For example, GetWell@Home enables hospitals to “provide an electronic copy of hospital discharge instructions on request”, “provide an electronic copy of health information to the patient upon request (including diagnostic test results, problem lists, medication lists, medication allergies, discharge summary and procedures)”, and send reminders to patients (per patient preference) for preventive and follow-up care”. GetWell@Home will become an even more important resource as Stage 2 and 3 standards, yet to be released, are expected to have even more stringent requirements for the exchange of information with patients over time. GetWell@Home will assist in satisfying these requirements and others securely and efficiently.
